Patients may discuss dissatisfaction or concerns about the quality or safety of patient care with any nurse, physician or Guest Relations at (815) 489-4008. Patients also may report concerns directly to the Illinois Department of Public Health by calling (800) 252-4343 or to The Joint Commission at (800) 994-6610.
