MySwedes Chart | Frequently Asked Questions


What is MySwedes Chart?

SwedishAmerican MySwedes Chart provides you with a secure, online connection to access portions of your medical record and manage your appointments. It offers a convenient way to communicate with your medical group provider on non-urgent healthcare matters.

What do I need to use MySwedes Chart?

You must be 12 years of age or older. You also will need access to a computer connected to the Internet and an up-to-date browser, such as Internet Explorer or Safari. In addition, you will need an email address for notifications and security verifications.

In what languages is MySwedes Chart available?

Currently, MySwedes Chart is available only in English. Future releases of MySwedes Chart may include other languages.

Is there a fee for this service?

No. MySwedes Chart is a free service for SwedishAmerican patients and families.

What if I need help?

Whom do I contact if I have further questions?

What medical information is available?

MySwedes Chart provides access to portions of your SwedishAmerican health information. This includes your allergies, immunizations, prescriptions, test results, diagnoses, visit summaries and instructions, appointments and past visits. MySwedes Chart does not display all information from your medical record. If you need information or services beyond what are available to you online, please contact your provider or MySwedes Chart Customer Service. Account Maintenance

How do I get my personal information corrected?

Health Insurance Portability and Accountability Act (HIPAA) permits patients to request amendments or corrections to their medical records in the event an inaccuracy is identified. Please contact the Privacy Office at (779) 696-7225 for information regarding the amendment process.

Request an Account (Account Registration)

How do I request an account?

You can request access to MySwedes Chart at your next visit to one of our SwedishAmerican Medical Group locations or ask your nurse before being discharged from the hospital.

What information do I need to provide?

Your provider will give you a Request for Access to MySwedes Chart Form to complete. If you are between the ages of 12–17, you also may sign up for MySwedes Chart. Please complete the Request for Access for Adolescents Form with your parent or legal guardian and submit ti to your provider’s office. You will need to provide the following information:

  • Photo ID
  • Legal name
  • Mailing address
  • Phone number
  • Last four digits of Social Security number
  • Private e-mail address
The information you provide is confidential and is processed through our secure system.

Do you offer access to other patient’s accounts?

Yes. This is called Proxy access. SwedishAmerican currently allows Proxy access to legal dependents ages birth through 11 years. To obtain Proxy access, you will need to request access in person at the clinic.

Appointment Information

What if I do not see an appointment listed?

If you do not see an appointment listed for an upcoming visit, please call your provider’s office for assistance.

Can I cancel an appointment?

Yes. You can send a cancellation request up to 24 hours before your appointment time. If your appointment occurs in less than 24 hours, contact your provider’s office for assistance.

Can I request an appointment?

Yes. You can request an appointment with a SwedishAmerican Medical Group provider whom you have seen before. Please allow 48 hours for processing and confirmation.

Billing Information

Can I pay my bill online?

Yes. Once you are logged into MySwedes Chart, click on the Pay My Bill link located on the home page to pay your hospital and clinic bill.

Can I view my statements online?

No. Currently, we do not offer electronic statements. A future release of MySwedes Chart will include this feature.


If I send a message to my provider, when can I expect a reply?

Generally, you will receive an answer within two business days. Please note that MySwedes Chart should not be used for urgent situations. If you experience a lifethreatening emergency, call 911.

Why do I need to supply an e-mail address?

We require that you provide an e-mail address so that you may receive your account activation and simple alerts. No protected health information is sent within these emails.

Test Results

When will I see my test results?

Approved test results will be released to your MySwedes Chart account within 3–7 days of their completion.

Why don’t I see all my test results?

Your provider will determine which types of test results are viewable in MySwedes Chart. Contact your provider with test result questions.

Proxy Access

What is Proxy access?

Access to another patient’s records is called Proxy access and SwedishAmerican currently allows proxy access to legal dependents ages birth through 11 years. To obtain Proxy access, you will need to request access in person at the clinic.

Why can’t I see my child’s record?

If you cannot access your child’s record, either Proxy access has not yet been established or there may be technical issues regarding your account. For assistance, please contact the MySwedes Chart Customer Service Line.

My child turned 12–why I can’t see his/her medical record?

In accordance with Illinois law, Proxy access through MySwedes Chart is permitted for legal dependents only ages birth through 11 years. Access automatically terminates on the child’s 12th birthday. However, you may request copies of your child’s records by contacting the Health Information Management Services (Medical Records) department or your provider’s office.

Problems Logging In

Why does my access code not work?

Your access code expires after 90 days and is valid only for one-time use. Upon activation this code is no longer valid. Your access code must match the exact spelling of your first and last name and date of birth on record at SwedishAmerican. For example, if you try to enter “Matt Smith” and the system lists you as "Matthew Smith," the registration will fail. If you have trouble registering your access code or need a new access code generated, please contact the Customer Service Line at (855) 641-5010.

What happens if I forget my username?

Visit the MySwedes Chart login page and click on Forgot your username. You will be prompted to enter your information. Your username will be e-mailed to you. If you are still having trouble, contact MySwedes Chart Customer Service Line at (855) 641-5010.

What happens if I forget my password?

Visit the MySwedes Chart login page and click on Forgot your password. You will be prompted to enter your information and answer your security questions. If you are still having trouble, contact MySwedes Chart Customer Service Line at (855) 641-5010.

Is my access code the same as my username?

No. Your access code is a one-time code used for your initial account generation. You created your username during your initial account setup. Visit the MySwedes Chart login page and click on Forgot your username.

Security Controls

What does “Session timed out” mean?

The system did not detect any activity in the MySwedes Chart site and ended your session. In order to reactivate your session you will need to log in by entering your username and password.

Why does the system time-out so fast?

To protect your information, your MySwedes Chart session will time-out after five minutes of inactivity.

How do I know my information is secure?

We take great care to make sure your health information is kept private and secure. SwedishAmerican MySwedes Chart uses secure encrypted communication. Access to your account is controlled through access codes, personal usernames and passwords. Please remember to maintain your password in a secure manner.

Secure Messaging

Is my message to my provider secure?

Yes. The messages sent from MySwedes Chart are handled by secure encrypted communication.

How will I know if I have a message from my provider’s office?

You can set up e-mail notifications under preferences. You will receive an e-mail when new messages have been added to your MySwedes Chart inbox.

Technical Questions

What are the Internet browser requirements?

MySwedes Chart requires:
  • Internet Explorer 9 or higher (with Compatibility View disabled), or the current versions of Firefox, Chrome or Safari.
  • Cookies enabled
  • JavaScript enabled

Is my mobile device supported?

Yes. If you currently have a MySwedes Chart account, you can access your health information from your tablet or mobile-ready device.
